Consent and Release Form
required for participation in all children's ministries
We are an Independent, Non-Denominational church.
We believe the KJV Bible is God's Word.
Our Purpose (Matthew 28:19-20):
1) To present the Gospel: We are all sinners (Romans 3:10). The price for sin is to die and spend eternity in hell (Romans 6:23). Jesus died to pay for our sin (Romans 5:8). Anyone who will call on Jesus and trust Him to be saved will be saved (Romans 10:13).
2) To teach the Bible: We teach how to have God's blessings on our life by obeying and living by the principles in the Bible.
.I understand the purpose and beliefs of GETHSEMANE CHURCH.
If my child(ren) has medical conditions which may be relevant to a physician in the event of an emergency, I have listed them below. In the event an emergency occurs, I may be reached at the telephone number(s) listed on the digital form used for registration. If I cannot be reached within a reasonable period of time, I hereby authorize the adult sponsor, Gethsemane Church Ministry Leader, to make emergency medical decisions for my child.
I understand and hereby agree to assume all of the risks which may be encountered while attending Gethsemane Church, including activities preliminary and subsequent thereto, as we as transportation to and from Gethsemane Church. If there are any activities that I do not wish for my child(ren) to participate in, I have written contacted the church through email or added to the registration form.
Gethsemane Church is hereby authorized to take or permit pictures to be taken of my child(ren), a minor(s), for use by Gethsemane Church in publications, newspapers, newsletters, its website, and/or on television for purposes of public relations for Gethsemane Church.
Exposure to COVID-19 is an inherent risk in any public location where people are present; we cannot guarantee you will not be exposed during your attendance. People who show no symptoms can spread Coronavirus if they are infected, and interaction with the general public poses an elevated risk of being exposed to Coronavirus. By coming to church, you acknowledge and agree that you assume these inherent risks associated with attendance.
I do hereby agree to hold Gethsemane Church and its agents and employees harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities.
I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of ILLINOIS. The terms of this release are contractual and not a mere recital.
I further state that I have carefully read the foregoing release and know the contents thereof, and have digitally signed this release as my own free act when I digitally signed the registration form . This is a legally binding agreement that I have read and understand.
Understanding this, I give my permission for the child(ren) listed on the church's registration for to ride the church bus, attend church, and be taught the Bible.
Bus Ministry Requirements Form
required for participation in the bus ministries
Dear Parent/Guardian,
We are thankful for your interest in Gethsemane Church and the Bus Ministry. Our goal is to provide a safe, orderly, and Christ-honoring environment for all children who ride our bus to and from church services and ministry activities.
Please carefully review the following requirements with your child.
Bus Ministry Guidelines

Pickup Time: Children should be ready between 5:45 PM and 6:45 PM.

Preparedness: Children must be dressed and have shoes on, with porch lights on for both pickup and drop-off.

Health: If your child is sick or contagious, please have them stay home.

Pickup/Drop-off Location: Children will be picked up and dropped off at the same location and parents can only confirm pickup for their own children.

Supervision: An adult must be present at drop-off to receive the child.

Transportation Responsibility: If a parent/guardian chooses to drop off their child, they are also responsible for picking them up unless communicated directly with an agent of the Bus Ministry Team.

Absence: If you have confirmed that your child is going but plans have changed and your child is no longer attending, please notify Zelma at 309-642-4327 ASAP.
Behavior Expectations
Your child is expected to be respectful in class and on the bus at all times. Good listening and appropriate behavior is necessary to maintain a safe and respectful environment.
Disciplinary measures for ignoring rules as follows :

First Warning: Verbal reprimand

Second Warning: Meeting with Pastor Noyes

Third Warning: Two-week suspension from class
We reserve the right to escalate disciplinary action at any time, depending on the severity of the behavior. This may include advancing directly to any stage outlined above, up to and including indefinite expulsion.
Cleanliness Policy
To help keep the bus clean, no food or drinks are allowed on the bus.
